Transitions care, the set of actions that to ensure the accurate continuity of health care when a patient is moved between different facilities or released to go home, is receiving a lot of attention. This is due to the fact that transfers are more dangerous than you realize.


When patients are moved, as their care needs change, they could switch among several types of providers. Moving from the hospital to a skilled nursing facility and then to home care creates many instances where coordination can go wrong.  The fact is, getting care from different providers, at different times, is not designed to keep patient safety in mind.
